Social Prescribing is a new NHS GP service that helps connect patients to a range of local, non-medical activities and opportunities through practical support. Social Prescribing Link Workers help people access specialists in • Benefits and debt advice • Counselling and other emotional support needs (bereavement, anxiety, depression) • Domestic abuse • Housing advice • Practical help (cooking, transport, shopping) • Maintaining independence at home at (equipment & telecare devices, Meals at Home, staying active) • Disability/impairment advice • and more
