Youth Social Prescribing

Our youth social prescribing team works with youths aged 11-17 years old to help and support you with any emotional health and wellbeing issues you may be experiencing. When you are referred to our service, we will meet with you, explore what is happening in your life and, if need be, signpost you to services and groups that may be of interest to you. We will also check in regularly to see how you are and make sure that you are getting all the support you need.

What does youth social prescribing involve?

  • Working with young people aged 11 – 17 years old
  • 1:1 appointments either at home, school or GP surgery
  • Following a holistic approach, asking “What matters to you?”
  • Providing a safe and confidential place to talk
  • Creating a personalised care plan based on individual needs
  • Advising and supporting referrals to other agencies e.g. counselling, CAMHs
  • Supporting with health and fitness through local gym referrals
  • Signposting and connecting individuals to activities, groups and services in their area to help support their physical, social and emotional wellbeing

Anyone who wants to explore support for their:

  • Low mood and/or anxiety
  • Low self esteem / self confidence
  • Emotions and stress levels
  • Social anxiety and loneliness
  • School concerns
  • Friendship or social difficulties
  • Body image, identity, health and fitness

  • GP’s and other health professionals
  • Other local support agencies and organisations
  • Schools and youth groups
  • Parents, carers and families

Our team of youth health and wellbeing coaches work with 11-17 year olds. We are here to support you with your health and wellbeing and help provide more in-depth guidance and support to achieve your goals.   • Focusing on developing understanding and taking responsibility

Who could benefit from youth coaching?

Anyone who wants to understand and better manage their:

  • Low mood and/or anxiety
  • Low self esteem / self confidence
  • Emotions and stress levels
  • Social anxiety, loneliness or friendship difficulties
  • School environment and key transitions
  • Individual identity and ability to achieve future goals

Coaching is not…

  • Therapy such as counselling/psychotherapy which explore past experiences, diagnose and treat emotional and psychological problems
  • Giving the answers or solutions

How can you access coaching?

All referrals for youth coaching come via the youth social prescribing team who can be accessed via your GP.


Youth Wellbeing Cafe

During the Summer holidays we will be running a ‘drop in’ cafe for any of our patients aged 11 – 17 years old that have accessed our social prescribing or youth coaching services.

If you would like somewhere to chill out, meet others, have fun or have a chat/appointment with one of our health and wellbeing team, please join us.

We will also be running a tuck shop for a small fee and, by popular request, will have a smoothie maker available.
